Case management
Our Case Management teams listen, advocate, support, and coordinate services with families and community supports, as well as professional support providers. The goal is to help individuals and families achieve self-sufficiency. Services include assessment and identification of mental health and psychosocial needs, an individualized plan developed with the child and family, and the implementation of that plan.

Case Managers work closely to monitor the plan and to make any necessary changes along the way. Our approach adapts to each family’s goals, culture, and resources.

Case Managers will work with the child and family to develop and achieve measurable goals and outcomes. Decisions, solutions, and problem-solving are achieved through collaboration, offering children and families thoughtful options for the best outcome.
